What is support coordination?

Coordination of Supports/Support Coordination is an NDIS funded component in a participants NDIS Plan. There are three levels Support Connection, Support Coordination and Specialist Support Coordination. Below is a description of each level.

Level 1 Support Connection

NDIS Support Connection is a service provided under the National Disability Insurance Scheme (NDIS) to assist participants in navigating and accessing supports and services available to them. Support Connection aims to help participants understand and utilize their NDIS plans effectively by providing guidance, information, and connecting them with appropriate service providers or community resources.

The role of a Support Connection provider is to work closely with the participant, their families, and support networks to identify their goals, needs, and aspirations. Level 2 Support Coordination

Level 3 Specialist Support Coordination

Specialist Support Coordination is an enhanced form of support coordination available under the National Disability Insurance Scheme (NDIS) for participants with complex needs or specific circumstances. It is designed to provide expert guidance and coordination for individuals requiring intensive support and assistance in navigating their NDIS plans.

Specialist Support Coordinators possess advanced knowledge and expertise in supporting participants with complex needs. They work closely with participants, their families, and support networks to develop a comprehensive understanding of their goals, challenges, and aspirations. They assess the participant’s unique circumstances and develop a tailored plan to address their specific needs.

The role of a Specialist Support Coordinator includes:

  1. In-depth Assessment: Conducting a thorough assessment to understand the participant’s strengths, limitations, and support requirements.
  2. Complex Support Planning: Developing a comprehensive plan that addresses the participant’s specific goals, taking into account their disability-related needs, health conditions, and other relevant factors.
  3. NDIS Plan Implementation: Assisting the participant in implementing their NDIS plan, including connecting them with appropriate service providers and coordinating the delivery of supports.
  4. Collaboration and Advocacy: Collaborating with relevant stakeholders, such as health professionals, housing providers, and community organizations, to ensure a coordinated and holistic approach to support delivery. Advocating for the participant’s needs and preferences within the NDIS system.
  5. Crisis Management: Providing support during crisis situations, such as transitions, changes in circumstances, or complex service coordination challenges.
