My Aged Care (MAC) is an Australian government initiative designed to streamline access to aged care services for older Australians. It provides information and support for individuals seeking aged care, whether it’s in-home care, residential care, or other support services.
A Home Care Package (HCP) is funded through the federal government My Aged Care to provide personalised support to older Australians who prefer to stay at home to help maintain independence and quality of life.
Are you eligible for a Home Care Package?
To qualify, you must be 65 or older (50 years or older for Aboriginal and/or Torres Strait Islander peoples) and need help with everyday tasks. Eligibility is assessed by My Aged Care, and is based on your health and wellbeing, mobility, independence and personal support networks
Who Is Trilogy Care
Trilogy Care supports older Australians in living well at home for longer by helping them build and self-manage the Home Care Package that suits their individual needs.
In partnership with Inspire Consulting Australia, the experienced Trilogy Care team works to maximise your care hours and enhance your wellbeing—offering you greater choice, control, and independence.
Our Partnership
Inspire Consulting Australia is a Home Care Package care coordination service. We partner with Trilogy Care, a registered Home Care Package provider, to coordinate and manage your Home Care Package.
Our role is to support you in managing your home care so you can continue to live confidently in your home.
We work with you to source services according to your personalised care plan. We can also liaise directly with Trilogy Care on administering your Home Care Package.
While you are working with your care coordinator, you have the freedom to contact your Trilogy Care care partner directly at any time or access your Home Care Package administration details via the Trilogy Care Portal.
